1. Make Contact

• For participants currently connected to Florida HIE, please complete this short Participant Transition Questionnaire so we can prepare your organization’s migration plan. 
• Once you submit the questionnaire, your new designated Florida HIE Account Manager will reach out to you directly. Please keep a look out for outreach from Rachel Teran or Kary Nulisch and reply once received.

2. Learn More

Once your questionnaire is completed and/or you receive direct outreach from AHCA or Florida HIE to begin your transition, you will be invited to an Onboarding Webinar to learn more about the migration to CRISP Shared Services’ technology infrastructure. During the webinar, we will review the following materials in more detail and answer any questions from participants.

Florida HIE and its partners, AHCA and CRISP Shared Services, host weekly Office Hours that are open for all participants. No registration is required. Please join us if you have any questions or want to discuss anything with our teams.

3. Sign Up

Below are copies of required forms for signature, depending on your organization’s explicit needs. These are for informational reference only. Your Account Manager will send you electronic forms for signature through DocuSign or Adobe Sign during the onboarding process. Please ensure you designate your organization’s legal/signatory representative in the Participant Transition Questionnaire.  

Note: All participants will need to complete the Participant Agreement, and your Account Manager will work with you to determine if any of the other agreements or forms need completion.
